Our legal team has prepared this health questionnaire to enable us to assess the extent to which people are experiencing health problems potentially attributable to radio frequency radiation exposure. If you have experienced any such problems and would like to submit evidence, you may do so either by completing the online form below, or by filling out a printed copy of the questionnaire document AA5G – EMF questionnaire – 07.09.20 and posting it to us at 70 Orme Road, WORTHING, West Sussex, BN11 4EX. We are collecting testimony for presentation to the Judicial Review at the High Court, and whilst all data gathered are held securely and subject to the provisions of the General Data Protection Regulation (GDPR), be aware that once presented as evidence to the High Court, submissions could potentially become a matter of public record.
